To get a Provisional Driving Licence, you will need to complete an application form known as Form D1. This form is available at most Post Offices or you can obtain one from the DVLA website: www.dvla.gov.uk
A Provisional Licence costs £50.00 (as of 01/04/08).
If you need to replace either the photo card or the paper part of your licence (or both parts), the cost is £17.50 (as of 01/04/08).
